As in all organizations, Pack 61 requires some paperwork to be filled out. Here's where you'll find all of our standard forms.
In order for scouts to participate in camping trips and events, their medical forms need to be completed by a parent or guardian (Parts A & B). Please fill out these forms and provide to the pack Secretary.
The pack does its best to let the community know about all the great things we do! And that usually means a photo is included in the local paper. We do not use names on the photo - just the reference to Pack 61 Cub Scouts in order to keep our kids safe. Bring this form to the next pack meeting and provide to the Secretary so we have your release on file.
